Off the top of my head, I can think of Accattone, Germania anno zero (which I saw yesterday for the first time in years) and Oldboy. Also 8 1/2 but that's more a feeling of joyous restlessness than shock and despondency, which is what I'm looking for.
What are some movie endings that make you feel like you've been kicked in the balls with steel-toed boots?

i'm not quite sure i understand what you're asking? the ending causes you great pain and brings you to your knees and that pain lingers for a lot longer than it should?

Big yes to Chinatown. I need to watch that one again, it's been ages. Didn't really care for The Fall.
I just remembered that I was hit pretty hard by The Devil and Daniel Johnston when I saw it in the BAFICI festival a few years ago. That really affected me somehow.

Well okay I'm not sure this is exactly what you're looking for, but...
I know this was definitely the only time in my life a viewing of a film ever left me this way. When I first saw RETURN OF THE KING in the theatre, it was shortly after opening, but I went on like the Mon or Tues early afternoon, so a fairly empty theater. And it's important to note that I'd never read the books at all, I fell totally in love with the whole story just on the movies alone, so I didn't know how it was all going to end.
I'll never forget how I felt walking out of there, in fact on the ride home I called my best female friend (she'd also been looking forward to seeing it). I mean I seriously felt physically ill. Like I'd been mentally and emotionally raped. I was completely wiped. Especially the final hour or so, it's like I experienced such a range of extreme emotions, yea... it really hit me.

If you mean, movies that kinda disturb me in the head, that movie 'Seven' fucked my head up big time for a day or so afterwards. I will never watch it again. It was that scene with the hooker and the guy has...yanno *shivers*.
