this flat out amazes me that they are even revisiting this event

The spacecraft broke up while re-entering Earth's atmosphere near the end of its mission on February 1, 2003.

The NASA report found the astronauts knew for about 40 seconds that they did not have control of the shuttle before they likely were knocked unconscious as Columbia broke apart around them. Watch more details from the report »

The report also found that while crew members were wearing their pressurized suits, one astronaut did not have on a helmet, three were not wearing gloves and none lowered the visors before the module lost cabin pressure. One astronaut also was not seated...

Live and learn. I guess the helmets all worked fine until something catastrophic happened. Faulty helmets or not, those astronauts weren't going to survive that no matter what.

They were on a ship that went 4th of July while reentering the atmosphere at thousand of miles per hour...damn those faulty helmets!

For real. If somebody told to manufacture a helmet that would save the life of an astronaut on orbit re-entry I'd tell them to go fuck themselves. How stupid.

This reminds me of a news report I saw a while back. Some kid sledded into a road and got run over by a truck. Obviously he died. The anchor ended the report with "Police have told wjz that the child was not wearing a helmet."
