-At the end of Ran, when the King's son gets killed essentially for no reason. The resulting scene is sickening, as well as the shot of the blind man alone on the cliff that follows.
-The first fifteen fabled minutes of Saving Private Ryan.
-The entire movie, pretty much, of Schindler's List.
-Brooks' suicide in Shawshank Redemption.
-Goebbels' wife poisoning all her children to death in Downfall.

Anyway, it is extremely rare that I find film hard to watch. The only things that come to mind are:
Deep Red - When the killer keeps smashing that guy's mouth onto the sharp edges of furniture. Ouch.
American History X - Curb stomp.

uh, the first 15 minutes of saving private ryan are fucking amazing. sometimes i turn on that scene just to watch it and then turn off the movie right after.
the only thing that has ever freaked me out was the curb stomp from american history x.

American History X- bite the curb
and in Taking Lives, when Ethan Hawke stabs Angelina Jolie in the pregnant belly with a pair of scissors. (the audience doesn't find out until afterwards that she isn't actually pregnant but just trapping him). i seriously COULD NOT take it.

I can see Requiem, Schindler's List and American History X's infamous curb stomp make people really squeamish (both still get to me) but really you guys have weak stomachs. Try:
Cannibal Holocaust: cutting open a live turtle. The movie was banned in Italy for animal cruelty.
Irreverisble: Basically the first 45 minutes of the movie.
Dogville: Not too bad and I would probably handle it better now but the last several scenes of the film and the treatment of Nicole Kidman really got to me.
Man Behind the Sun: The entire film is a gorefest, but to fully understand that it was actually based on TRUE events is terrifying. Really shows the sick side of humanity during war. Very, very affecting film despite boardering on exploitation.
Ichi the Killer: Some people call it art, I call it a journey into the depraved.
Braveheart: The throat slitting scene is probably the best I've ever seen in a movie

I found that movie to be very, very affecting, but not necessarily hard to watch. It looked incredibly realistic, but none of it made me cringing in agony. Although that stick-up-the-vagina scene was pretty brutal.
And Ichi The Killer didn't bother me at all. Sure, it's depraved and one of the most insanely violent films ever made, but it's also so goddamn hilarious that I was too busy laughing the whole time to be disturbed.
